2018 Scholarship Application

Basic Application Criteria
Polk County Public and Charter High School Seniors must:
  • Be in line to graduate, specifically having passed the Algebra I EOC (or have the concordant math PERT score of 97) AND by having passed the FSA reading (or have the concordant ACT score of 19 or SAT reading of 430)

    Students from Ridge and Traviss Career Centers, East and West Area Adult Schools and Gause Academy are not required to have these test scores.

  • Have FAFSA (Free Application for Federal Student Aid) completed your SAR (Student Aid Report) ready to attach to this application (FASFA will not be accepted, only the SAR)

  • Review the list of scholarships to see what you are eligible to apply for as each award has its own minimums. This list personalized for you will be displayed after you have entered and saved the basic application items. New in 2018, there is no minimum GPA to start an on-line application.

2018 Scholarship Application Directions
  • Gather all required information before you start.
    You'll need to have:
    • your SAR (Student Aid Report) with EFC (Estimated Family Contribution)
    • dates for your work history (if applicable)
    • list of scholarships already awarded and
    • a prepared essay.

  • To START a new application, log in on screen one of the application by clicking "Start New Application" after entering your student ID and date of birth. You will be able to set a password to login at a later time to access your saved application. An email address is required for the password recovery feature. The PEF may also use this email address to contact you with questions or interview requests.

  • Once you enter your student ID number, the following information will be pulled from your permanent record and can't be changed:

    Name
    School
    Weighted GPA
    Passage of Alg 1 EOC and FSA Reading
    SAT and or ACT scores
    Volunteer hours
    The following two items will also be pulled from your permanent records, but you can make changes to update these fields:
    mailing address
    phone number
    (It is very important that your contact information is correct, if you are a scholarship recipient, it is used to communicate with you!)

  • To RETURN to an application at a later time, log in on screen one of the application by clicking "Login to Saved Application" after entering your ID and password. If you forget your password can be emailed to the address you provided when you started your application using the 'forgotten your password' link at the bottom of the Login page.
    HINT: The website will not allow you to start a second application once a student ID is in use, you must go back and make changes to the first saved application. If you get the message "student ID already in use", you must enter as a returning applicant.

  • Complete Your Application.
    • As you finish each page hit the "save application" button at the bottom of the page to ensure you do not lose data, then hit "continue" to move forward to a new page.
    • Run out of time to complete it all? Click "save application" then "quit & close session" and you can return at a later time using the directions above.
    • Read the criteria for each award carefully. If you do not meet ALL the requirements, do not apply for that award. DO NOT check every award without first seeing if you qualify!
    • Once you have completed all sections of the application, then and only then should you hit the "submit" button on the final screen of the application. Once you hit "submit" you cannot go back and make any changes to your application. You should print your application at time of submission. If you forget you may log back on to print, however no changes can be made to your application. .

  • Even though the application is submitted electronically, to be considered YOU MUST print out your completed application and attach your SAR (Student Aid Report).

    A listing of all classes taken in high school will automatically print as part of your application, it is not required for the school to submit transcripts as part of your application package. Once your application is printed, it must be signed by the student, and if the student is not 18, also signed by their parent or guardian.

Turn in all materials to your Guidance Counselor / scholarship contact by the deadline posted in their office.

Check with your school's Guidance Counselor/scholarship contact for your school's deadline prior to the February 6 closing of this application so you are not late with your hard copy submission to them.

Please Note: The application website will expire on Tuesday, February 06, 2018 at noon.

REMINDER: You must print a copy of your application, sign it (and obtain parent or guardian signature if you are not 18), and attach your SAR report. The pack will then be due to your Guidance Counselor/scholarship contact before their specific deadline. Once again, submitting online does not qualify your application; you must print and submit all required hard copy items to be considered.

All scholarship recipients will be announced at their high school's senior award ceremonies; no letters of regret will be sent.

Recipients will receive letters of congratulations and instruction from the Polk Education Foundation (PEF) after all senior award ceremonies are complete. Recipients will be given the name and address of their scholarship sponsor and will be required to send them a thank you. A copy of that note along with a completed "College Information Sheet" (this will be enclosed with the award letter) must be submitted to the PEF for funds to be disbursed to the school of the students' choice.

Scholarship funds are held at the Polk Education Foundation for students use for up to five years, and then if unused, they are returned to the donor fund.

Select scholarships require transcripts to be submitted annually for renewal, this will be marked on a recipient's award letter. Students must pay close attention to those terms and make submissions each year for renewals to be paid.
